Immunization Requirement Kindergarten thru 5th grade

New York State Public Health Law requires that all students be up to date on age-appropriate vaccinations in order to attend school. These vaccines protect children from serious illnesses and help maintain a safe and healthy school environment for everyone.

The required immunizations vary by age and grade level but typically include vaccines for:

  • Diphtheria, Tetanus, and Pertussis (DTaP/Tdap): Required for school attendance, including booster doses for immunity.
  • Hepatitis B: A series of vaccinations for this virus is required.
  • Measles, Mumps, and Rubella (MMR): Protects against these three viral diseases and is mandatory for enrollment.
  • Polio: A series of polio vaccinations (IPV) is required to attend school.
  • Varicella (Chickenpox): A vaccine for chickenpox is required for school entry.

Please ensure your child’s immunization records are current and submit documentation to the school nurse or health office before the first day of school. If your child is missing any required vaccines, they must receive at least the first dose before school begins to be allowed to attend.

Immunization Requirement for 6th grade

Students entering 6th grade in NYC public schools, must have a booster dose of the Tdap vaccine (Tetanus, Diphtheria, Pertussis), which is required for children 11 years or older

Children in grades 6 to 12 must also meet the requirements for the
tetanus, diphtheria and pertussis (Tdap) booster and meningococcal
conjugate (MenACWY) vaccine.

Specific Requirements for 6th Grade 
  • Tdap Vaccine: Students in grades 6-12, who are 11 years or older, must have one dose of the Tdap vaccine.
Other Required Vaccines for School Attendance 
  • MMR (Measles, Mumps, Rubella): Two doses are required for students in grades K-12.
  • Polio: Four doses are generally required for Pre-K through 12th grade.
  • Hepatitis B: Three doses are required for Pre-K through grade 12.
  • Varicella (Chickenpox): Two doses are needed, or a doctor's note confirming the child has had the disease.
  • Diphtheria/Tetanus/Pertussis: Students need a certain number of doses depending on their birth year, with an 11-year-old's booster being the Tdap.

If your child has already received the vaccine, kindly provide documentation from your healthcare provider. If not, please schedule an appointment as soon as possible and submit a note indicating that the vaccine will be administered.

Immunization Requirement for 7th grade

New York State law mandates that all students entering 7th grade must receive at least one dose of the meningococcal conjugate vaccine (MCV4). This vaccine helps protect against meningococcal disease, a serious bacterial illness that can lead to meningitis and other life-threatening infections.

Children in grades 6 to 12 must also meet the requirements for the
tetanus, diphtheria and pertussis (Tdap) booster and meningococcal
conjugate (MenACWY) vaccine.

Specific Requirements for 7th Grade

Meningococcal vaccine: One dose is required for students entering 7th grade.

Tdap vaccine: Students entering 7th grade must have already received one dose of the Tdap (Tetanus, Diphtheria, and Pertussis) vaccine.

Other Required Vaccinations for School Attendance

In addition to the 7th-grade specific requirements, all students in NYC are generally required to be up to date on the following vaccines for school entry:

  • Hepatitis B vaccine
  • Measles, Mumps, and Rubella (MMR) vaccine
  • Polio vaccine
  • Varicella (chickenpox) vaccine
